Roles & Responsibilities

Provide compassionate, patient-centered care that improves daily life
Administer medications and treatments safely and accurately per orders
Document patient assessments, interventions and responses clearly and promptly
Collaborate with RNs, providers and the care team to deliver coordinated care
Monitor patient status and report changes to the clinical team in a timely manner
Support resident comfort and dignity through skilled bedside care and communication

Qualifications

Active LPN license in New York (required)
Completion of an accredited practical nursing program
Current BLS/CPR certification or willingness to obtain upon hire
Prior LPN experience preferred; new grads with strong clinical skills are welcome to apply
Strong communication skills and the ability to work well in a team-focused setting
Reliable professional references and clear background/health screenings as required

Education

Diploma or certificate from an accredited Practical Nursing program (LPN)

Knowledge & Skills

Solid understanding of basic nursing fundamentals and medication administration
Familiarity with patient charting and documentation best practices
Ability to perform vital signs, wound care and routine nursing tasks confidently
Good clinical judgment and the capacity to escalate concerns appropriately
Compassionate bedside manner with strong patient and family communication skills
