Roles & Responsibilities

• Provide compassionate, hands-on physical therapy interventions to SNF residents under the direction of the supervising PT
• Implement therapeutic exercise, gait training and mobility programs to help patients regain independence
• Assess and document patient progress clearly and timely in the patient record
• Work closely with nurses, aides and interdisciplinary team members to coordinate plan of care
• Educate patients and families on home programs, safety and fall prevention
• Maintain therapy equipment, ensure infection control and follow facility policies
• Participate in care planning, discharge planning and team meetings to optimize outcomes

Qualifications

• Licensed Physical Therapist Assistant (PTA) in Arizona or eligible for Arizona licensure
• 1-3 years of PTA experience, SNF or rehabilitation experience preferred
• Current BLS/CPR certification required or willing to obtain
• Comfortable with documentation in electronic health records and meeting productivity expectations
• Strong communication skills and a collaborative attitude

Education

• Associate degree from an accredited Physical Therapist Assistant program or equivalent
• Graduation from an accredited PTA program and successful completion of required licensure exams

Knowledge & Skills

• Clinical skills in therapeutic exercise, gait training and transfers
• Skilled in patient assessment, treatment planning and progress documentation
• Knowledge of SNF/regulatory requirements and skilled nursing workflows
• Familiarity with mobility aids, adaptive equipment and safe patient handling
• Good time management, prioritization and teamwork skills
• Ability to communicate effectively with patients, families and interdisciplinary teams
