Roles & Responsibilities

• Evaluate residents’ functional mobility and develop individualized treatment plans
• Deliver skilled therapeutic interventions to improve strength, balance, and daily function
• Document treatment progress clearly and communicate outcomes to the interdisciplinary team
• Collaborate with nursing and rehab staff to ensure safe transitions and discharge planning
• Educate residents and families on therapeutic goals and home exercise programs

Qualifications

• Licensed Physical Therapist in Texas (or ability to obtain Texas licensure promptly)
• Current Basic Life Support (BLS) certification (or willing to obtain)
• Experience in post-acute, skilled nursing, or rehab settings preferred
• Strong clinical assessment and documentation skills
• Excellent communication and teamwork skills; patient-centered approach

Education

• Degree in Physical Therapy (DPT, MPT, or equivalent) preferred

Knowledge & Skills

• Knowledge of rehabilitation principles for adult and geriatric populations
• Skilled in mobility training, gait training, transfer techniques, and therapeutic exercise
• Familiarity with state documentation and compliance standards for skilled nursing
• Ability to prioritize caseload and adapt treatments to individual resident needs
• Comfort collaborating within a multidisciplinary care team
