Roles & Responsibilities

• Evaluate speech, language, cognitive-communication, and swallowing needs for SNF residents
• Develop and implement individualized treatment plans focused on functional outcomes
• Deliver evidence-based therapy sessions and adapt interventions to resident progress
• Document evaluations, progress notes, and discharge summaries according to facility and CRG policies
• Collaborate with nurses, CNAs, therapists, families, and the interdisciplinary team to optimize care
• Educate residents and families on carryover strategies to maximize independence and safety
• Meet productivity expectations (therapist productivity requirement: 90%) while maintaining high-quality care
• Follow facility policies and represent the therapy team professionally (black scrubs dress code)

Qualifications

• Professional experience as an SLP (listed requirement: 1–3 years experience)
• Comfort working in a skilled nursing facility (SNF) practice setting
• Ability to be on-site to receive guaranteed hours and meet scheduling expectations
• Willingness to follow employer policies (CRG P&P) and meet productivity standards
• Strong interpersonal skills to collaborate effectively with residents, families, and multidisciplinary teams

Knowledge & Skills

• Skilled in SLP clinical assessment and treatment for adult SNF populations
• Competent documentation skills and familiarity with productivity tracking
• Strong communication and patient/family education abilities
• Ability to collaborate within an interdisciplinary team
• Good time management and organizational skills to balance caseload and documentation
