Roles & Responsibilities

• Perform thorough PT evaluations to identify patient needs and set meaningful goals
• Deliver daily skilled treatments focused on restoring function and independence
• Plan and execute safe, effective discharge plans to support rehab-to-home outcomes
• Collaborate with interdisciplinary rehab team to optimize patient progress
• Document care accurately and promptly in accordance with facility policies
• Meet productivity expectations while maintaining high-quality, patient-centered care

Qualifications

• Current PT license (able to practice in Missouri) — DPT preferred
• 1-3 years of clinical experience (SNF experience strongly preferred)
• Comfortable working in a fast-paced skilled nursing environment
• Able to meet productivity expectations (90% productivity requirement)
• Willingness to follow CRG policies & procedures and facility dress code (black scrubs)
• Strong interpersonal and communication skills for patient and team interactions

Education

• Doctor of Physical Therapy (DPT) preferred or entry-level PT degree and current PT license

Knowledge & Skills

• Skilled in PT evaluations, treatment planning, and discharge planning
• Experience with geriatric/medically complex patients common in SNF settings
• Good clinical judgment and ability to prioritize caseload effectively
• Ability to work collaboratively with nursing and rehab teammates
• Basic familiarity with documentation standards and productivity tracking
