Roles & Responsibilities

• Evaluate residents to develop individualized, evidence-based PT plans of care
• Design and implement therapeutic interventions to improve mobility, strength, and function
• Document treatment progress clearly and timely to support continuity of care
• Collaborate with nursing, OT, SLP and interdisciplinary team members to coordinate optimal care
• Educate residents and families on safe mobility, fall prevention, and therapeutic goals
• Participate in care planning and discharge planning to support successful transitions

Qualifications

• Doctor of Physical Therapy (DPT) degree or equivalent physical therapy degree
• Licensed Physical Therapist (PT) in Minnesota or eligibility to obtain state licensure
• Comfortable working in a Skilled Nursing Facility (SNF) setting; new graduates are welcome
• Strong assessment and manual therapy skills, plus solid clinical reasoning
• Good communication and teamwork skills to work effectively with interdisciplinary staff

Education

• Doctor of Physical Therapy (DPT) or equivalent degree in Physical Therapy

Knowledge & Skills

• Skilled in patient assessment, mobility training, gait training, and therapeutic exercise
• Knowledge of SNF regulations, care planning, and documentation practices
• Ability to set measurable goals and track outcomes
• Strong interpersonal skills for resident and family education
• Comfortable using electronic medical records for documentation
