Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments safely and accurately
Monitor and document changes in patient conditions and report concerns promptly
Collaborate with RNs and the interdisciplinary care team to deliver coordinated care
Assist with daily living activities, admissions, transfers, and discharges as needed
Maintain accurate clinical documentation and medication records in the patient chart
Support a positive, respectful environment that promotes dignity and comfort

Qualifications

Active Licensed Practical Nurse (LPN) license in New York (required)
Current BLS/CPR certification (or willingness to obtain)
Previous LPN experience in long-term care, skilled nursing, or related setting preferred
Strong communication skills and the ability to work well in a team
Ability to pass background check and any required health screenings

Education

Completion of an accredited Practical Nursing / LPN program
High school diploma or GED (standard prerequisite)

Knowledge & Skills

Medication administration and safe documentation practices
Basic wound care and vital signs monitoring
Familiarity with infection control and patient safety protocols
Experience with electronic medical records (EMR) is a plus
Good time management, organization, and patient advocacy skills
