Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to adults in the Uptown Health/Rehab area
• Administer medications and treatments safely and accurately
• Conduct timely patient assessments and monitor changes in condition
• Document care clearly and thoroughly in the patient record
• Collaborate with the interdisciplinary care team to coordinate effective treatment plans
• Educate patients and families about care plans, medications, and discharge instructions
• Adhere to facility policies, infection control and safety standards to protect patients and staff

Qualifications

• Active LPN or RN license (appropriate to the role applied for) in good standing
• Clinical experience in long-term care, rehabilitation, or similar settings preferred
• Comfort working the posted day shift hours (6:00 AM - 6:30 PM); scheduling details to be confirmed
• Current basic life support (BLS) certification preferred or obtainable upon hire

Education

• Graduation from an accredited LPN or RN nursing program (as applicable for the role)

Knowledge & Skills

• Strong medication administration and documentation skills
• Solid patient assessment and clinical judgment
• Comfortable collaborating with interdisciplinary teams
• Excellent communication and time management in a fast-paced clinical setting
• Compassionate bedside manner and commitment to patient dignity
