Roles & Responsibilities

• Evaluate and treat residents in a skilled nursing facility setting using evidence-based PT practices
• Develop individualized treatment plans that promote functional independence and quality of life
• Collaborate with nursing and interdisciplinary teams to coordinate resident care
• Document assessments and progress thoroughly and timely to meet facility standards
• Maintain productivity expectations (90% productivity requirement) while delivering high-quality care
• Adhere to CRG policies and procedures and represent the facility professionally (black scrubs dress code)
• Be present on-site to receive guaranteed hours and participate in team-based care

Qualifications

• PT (Physical Therapist) or DPT (Doctor of Physical Therapy) — credentials listed in the posting
• 1-3 years of clinical experience (posting specifies Years Experience: 1-3)
• Comfort working on-site in a skilled nursing facility (SNF) setting
• Willingness to follow CRG policies & procedures and meet productivity expectations
• Professional communication skills for coordinating with residents, families, and the interdisciplinary team

Education

• DPT or PT degree (as indicated by PT / DPT in the posting)

Knowledge & Skills

• Experience and comfort treating geriatric and skilled nursing populations
• Skilled in functional mobility, balance, gait training, transfers, and ADL-focused therapy
• Strong documentation habits and familiarity with productivity tracking
• Teamwork and communication within interdisciplinary care teams
• Ability to work efficiently while maintaining patient-centered care
