Roles & Responsibilities

• Conduct thorough patient evaluations to create targeted treatment plans
• Provide daily treatments that improve patient mobility and independence
• Manage discharge planning to ensure safe transitions back to the community
• Collaborate with the interdisciplinary rehab team to deliver coordinated care
• Maintain timely, accurate documentation and meet productivity expectations
• Follow Continuum Rehabilitation Group policies and facility protocols
• Be present onsite to receive guaranteed hours and support patient needs

Qualifications

• PT license (Physical Therapist) — DPT preferred
• 1-3 years of experience (as listed) with strong SNF/long-term care experience preferred
• Comfortable working in a skilled nursing facility setting and following CRG policies
• Ability to meet productivity requirements (90%) and reliable onsite presence
• Professional communication skills and ability to collaborate with nursing and rehab staff

Education

• DPT preferred; degree in Physical Therapy or equivalent clinical training

Knowledge & Skills

• Patient evaluation and functional assessment best practices
• Skilled daily treatment planning and therapeutic interventions
• Discharge planning and care transition coordination
• Skilled nursing facility (SNF) workflows and documentation standards
• Time management, productivity tracking, and meeting performance goals
• Strong interpersonal skills for working within a rehab team
