Roles & Responsibilities

• Provide skilled speech-language pathology evaluation and treatment for residents in a skilled nursing facility
• Develop individualized treatment plans and document progress in accordance with facility and CRG policies
• Collaborate closely with the interdisciplinary team to coordinate resident care and discharge planning
• Maintain required productivity standards (90% productivity requirement) while delivering high-quality care
• Follow CRG policies & procedures and facility-specific protocols
• Be present on-site during scheduled hours to receive guaranteed hours and participate in team activities
• Maintain professional appearance and adhere to dress code (black scrubs)

Qualifications

• 1-3 years of SLP experience (as listed in the job details)
• Experience working in Skilled Nursing Facility (SNF) or similar post-acute care settings preferred
• Current SLP credentials and state licensure in New Mexico, or eligibility to obtain licensure (confirm facility requirements)
• Ability to meet productivity requirements and follow CRG policies & procedures
• Strong documentation skills and ability to work collaboratively with a therapy team

Knowledge & Skills

• Skilled in speech-language pathology assessment and treatment for adult/geriatric populations
• Familiarity with SNF therapy workflows and post-acute care documentation
• Good clinical reasoning and treatment planning abilities
• Strong communication and interdisciplinary collaboration skills
• Comfort with productivity expectations and time management in a busy therapy environment
