Roles & Responsibilities

• Assess patients and perform comprehensive evaluations to identify functional limitations
• Develop individualized treatment plans that focus on safe, measurable progress
• Deliver skilled physical therapy using evidence-based techniques and therapeutic modalities
• Document patient progress, modify plans of care, and communicate changes clearly
• Collaborate with nurses, therapists, physicians and family members to coordinate care
• Educate patients and caregivers on home programs, safe mobility and fall prevention
• Participate in discharge planning to ensure smooth transitions and optimal outcomes

Qualifications

• Active or eligible Physical Therapist license in California
• Current BLS/CPR certification
• Clinical experience in rehabilitation or outpatient/inpatient therapy preferred
• Strong documentation skills and comfort with electronic medical records
• Excellent communication and teamwork skills; ability to engage patients and families
• Ability to work the stated daytime schedule including weekend coverage as required

Education

• Doctor of Physical Therapy (DPT) preferred or Master’s in Physical Therapy

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy and gait training
• Knowledge of orthopedic and neurologic rehabilitation principles
• Familiarity with assistive devices and safe transfer techniques
• Strong clinical reasoning, outcome measurement and goal-setting skills
• Comfortable using EMR systems and maintaining timely documentation
• Patient-centered approach with strong interpersonal skills
