Roles & Responsibilities

• Provide compassionate direct care including ADLs, feeding, mobility support, and timely response to call lights
• Assist with ambulation, transfers, and positioning using gait belts, non-mechanical aids, and mechanical lifts
• Support mealtimes with setup, feeding assistance, and accurate documentation of intake
• Follow individualized care plans and communicate changes in resident status to the licensed nurse
• Document ADLs, intake/output, restorative care, and other tasks in the electronic charting system prior to end of shift
• Maintain a clean, safe, and organized resident environment and report safety or equipment concerns immediately
• Participate in onboarding and training of new staff and contribute to a calm, home-like atmosphere
• Follow infection control protocols, including PPE use, hand hygiene, and transmission-based precautions

Qualifications

• Active BLS required at time of submission
• Current certification as a Certified Nursing Assistant on the Wisconsin Nurse Aide Registry (WI)
• 2+ years of relevant CNA experience
• Ability to lift 50+ lbs and support residents safely
• Computer literacy with experience or comfort using Electronic Medical Records (EMR)
• Willingness to float across units including long term care, rehab, dementia, and ventilator unit
• Comfort working in a 1:8–10 resident ratio and participating in weekend/holiday rotations

Knowledge & Skills

• ADLs, transfers, ambulation and safe use of gait belts
• Mechanical lifts (sit-to-stand, full-body) and safe body mechanics
• Infection control practices and PPE use
• Restorative and range-of-motion exercises
• Feeding assistance and nutritional safety monitoring
• Accurate EMR documentation and charting prior to end of shift
• Effective, empathetic communication with residents, families, and interdisciplinary teams
• Awareness of ventilator care basics, dementia care, and end-of-life/hospice support
