Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Conduct thorough start-of-care visits and prioritize them as required
- Supervise Physical Therapy Assistants to ensure high-quality patient care
- Complete clinical documentation in Kinnser within 48 hours of each visit
- Report start-of-care visits to the team the next day as part of mandatory reporting
- Maintain timely communication with your supervisor, intake, and office teams
- Develop and implement patient-centered care plans and assessments in the home environment
- Manage a caseload that meets productivity expectations while keeping patient care front and center
Qualifications
- Current CA Physical Therapist (PT) license — required
- Minimum 1 year of Home Health experience — required
- Comfortable with clinical documentation systems (Kinnser experience preferred)
- Experience performing start-of-care visits and home health evaluations
- Able to supervise PTAs and provide clinical oversight
- Willingness to travel within Solano County and expand territory as needed
- Responsive communicator during normal business hours
Knowledge & Skills
- Home health clinical assessment and care planning
- Kinnser documentation workflows and timely charting practices
- OASIS, recertification, resumption of care, discharge, evaluation, and revisit processes
- Start-of-care prioritization and mandatory reporting
- Patient communication and home safety assessment
- Caseload management and meeting productivity expectations (point-based system)
- Comfort using mobile devices (iPad) and field communication tools (Verizon One Talk)
