Roles & Responsibilities

• Provide patient-centered physical therapy in the home setting, tailoring plans to individual needs
• Perform thorough assessments and develop measurable treatment plans to restore function and independence
• Complete 5–6 patient visits per day on average while maintaining high-quality documentation
• Document all care using HCHB and ensure timely, accurate clinical notes
• Coordinate with nursing, hospice, and the interdisciplinary team to optimize patient outcomes
• Educate patients and caregivers on home exercises, safety, and fall prevention
• Manage your caseload efficiently and communicate proactively about any barriers to care
• Maintain a reliable vehicle and professional approach for safe, on-time patient visits

Qualifications

• Minimum 2 years of clinical experience (home health experience for therapy positions preferred and requested)
• Licensed Physical Therapist in the state of practice (state licensure required)
• Familiarity with HCHB documentation or willingness to learn quickly
• Comfortable delivering care in patient homes with strong independent decision-making
• Reliable vehicle and valid driver’s license for patient visits in Franklin / Macon County

Knowledge & Skills

• Strong knowledge of home health physical therapy practices and hospice considerations
• Excellent documentation skills, especially using electronic charting (HCHB)
• Skilled in patient assessment, mobility training, gait and balance interventions
• Time management and route planning to maximize patient contact and minimize drive time
• Clear communication and family/caregiver education abilities
• Ability to collaborate with interdisciplinary teams and adapt to varied home environments
