Roles & Responsibilities
- Conduct comprehensive psychosocial assessments that identify strengths, needs, and treatment priorities
- Develop individualized treatment plans grounded in evidence-based practice
- Deliver therapeutic interventions across the lifespan to address depression, anxiety, trauma, substance use, and adjustment disorders
- Collaborate closely with primary care providers, nurses, and behavioral health colleagues as part of an integrated care team
- Coordinate care and referrals to ensure patients receive holistic, whole-person services
- Maintain timely, clear, and compliant clinical documentation
- Provide culturally sensitive, trauma-informed care tailored to rural and underserved populations
- Participate in team meetings and care planning to improve patient outcomes and clinic workflows
Qualifications
- Active LISW license in Iowa (required)
- Master's degree in Social Work (MSW) from an accredited program (required)
- Minimum of 2 years of clinical experience preferred
- Experience working in rural or medically underserved settings is a plus
- Strong interpersonal and organizational skills with excellent clinical documentation
- Comfort collaborating in an integrated primary care/behavioral health team
Education
- Master's degree in Social Work (MSW) from an accredited program
Knowledge & Skills
- Expertise in psychosocial assessments and treatment planning
- Skilled in evidence-based therapeutic interventions (trauma-informed care, CBT, etc.)
- Experience treating depression, anxiety, substance use, and adjustment disorders
- Proficient clinical documentation and case management skills
- Care coordination and interdisciplinary collaboration
- Ability to engage patients across the lifespan and adapt interventions to rural populations
