Roles & Responsibilities

• Provide compassionate direct care including feeding assistance, mobility support, and personal care
• Assist with ambulation, transfers, and use of gait belts and mechanical lifts to ensure resident safety
• Monitor and report changes in residents’ physical, emotional, and cognitive status to the licensed nurse
• Document resident care, ADLs, intake/output, and restorative activities accurately in the electronic medical record prior to end of shift
• Follow individualized care plans and deliver care that honors residents’ preferences, rights, and safety
• Respond promptly and kindly to call lights and resident needs
• Support dining activities, assist with adaptive devices and prosthetics, and help ensure nutritional safety
• Maintain a clean, safe, and organized resident environment, including infection control practices and PPE use
• Participate in care conferences, onboarding of new staff, and emergency response procedures
• Support residents in specialized units including long term care, rehab, dementia care, and ventilator care with professionalism and empathy

Qualifications

• Current certification as a Certified Nursing Assistant on the Wisconsin Nurse Aide Registry (required)
• Active Basic Life Support (BLS) certification at time of submission (required)
• Minimum 2+ years of CNA experience
• Ability to lift 50+ lbs and safely assist with transfers
• Computer literacy including experience with electronic medical records
• Willingness to work in all units including long term care, rehab, dementia, and vents; refusal to work specific units may lead to contract cancellation
• Reliable, compassionate, and professional communication with residents, families, and interdisciplinary staff

Education

• High school diploma or GED preferred
• Completion of an accredited CNA program
• Active CNA certification on the Wisconsin Nurse Aide Registry
• Current BLS certification

Knowledge & Skills

• Patient care and activities of daily living (ADLs) support
• Safe transfer and mobility techniques, including sit-to-stand and full-body lifts
• Feeding assistance and monitoring intake
• Vital sign monitoring and basic observation skills
• Electronic Medical Records documentation and basic computer skills
• Infection control, PPE, and transmission-based precautions
• Effective communication with residents, families, and clinical teams
• Restorative care and range-of-motion support
