Shift Type: Rotating 12-hour day shifts, 6:00 AM - 6:00 PM, schedule varies with every other weekend required and every other holiday required (day shift)
Hours per Week: 48
Candidate Pay Rate: $47 - $32.9 per hour

Roles & Responsibilities
- Provide compassionate direct care including feeding assistance, mobility support, and personal care
- Assist with ambulation, transfers, and use of gait belts and mechanical lifts to ensure resident safety
- Monitor and report changes in residents’ physical, emotional, and cognitive status to the licensed nurse
- Document resident care, ADLs, intake/output, and restorative activities accurately in the electronic medical record prior to end of shift
- Follow individualized care plans and deliver care that honors residents’ preferences, rights, and safety
- Respond promptly and kindly to call lights and resident needs
- Support dining activities, assist with adaptive devices and prosthetics, and help ensure nutritional safety
- Maintain a clean, safe, and organized resident environment, including infection control practices and PPE use
- Participate in care conferences, onboarding of new staff, and emergency response procedures
- Support residents in specialized units including long term care, rehab, dementia care, and ventilator care with professionalism and empathy
Qualifications
- Current certification as a Certified Nursing Assistant on the Wisconsin Nurse Aide Registry (required)
- Active Basic Life Support (BLS) certification at time of submission (required)
- Minimum 2+ years of CNA experience
- Ability to lift 50+ lbs and safely assist with transfers
- Computer literacy including experience with electronic medical records
- Willingness to work in all units including long term care, rehab, dementia, and vents; refusal to work specific units may lead to contract cancellation
- Reliable, compassionate, and professional communication with residents, families, and interdisciplinary staff
Education
- High school diploma or GED preferred
- Completion of an accredited CNA program
- Active CNA certification on the Wisconsin Nurse Aide Registry
- Current BLS certification
Knowledge & Skills
- Patient care and activities of daily living (ADLs) support
- Safe transfer and mobility techniques, including sit-to-stand and full-body lifts
- Feeding assistance and monitoring intake
- Vital sign monitoring and basic observation skills
- Electronic Medical Records documentation and basic computer skills
- Infection control, PPE, and transmission-based precautions
- Effective communication with residents, families, and clinical teams
- Restorative care and range-of-motion support
