Shift Type: Night shift, 6:45 PM - 7:15 AM (36-hour guarantee); specific days scheduled by facility
Hours per Week: 36
Candidate Pay Rate: $48 per hour

Roles & Responsibilities
- Administer medications and perform treatments within the scope of LPN practice, safely and accurately
- Assess patients/residents and report changes, reactions, and observations to the supervising RN
- Collaborate with the RN to develop and implement individualized nursing care plans
- Provide patient and family teaching to support understanding and recovery
- Maintain accurate, timely documentation in the medical record (Epic EMR)
- Assist with emergency measures and respond effectively to sudden changes in condition
- Uphold facility and Nursing Department policies, including resident rights and long-term care standards
- Work as an engaged team member, accepting assignments and contributing to a positive care environment
Qualifications
- Active Michigan LPN license required (must be verifiable via LARA/Nursys)
- Active American Heart Association BLS (AHA) required
- 2+ years of Long Term Care (LTC) experience required
- Current skills checklist (to within 1 year) pertinent to LTC
- Two supervisory reference forms completed within the last two years
- Experience with Epic EMR preferred
- Candidates must provide permanent address; facility will not consider local candidates within a 100-mile radius
Education
- Graduation from an accredited Practical Nursing program and LPN certification/licensure
Knowledge & Skills
- Medication administration and treatment procedures
- Patient assessment and monitoring
- Care plan implementation and documentation
- Emergency response and basic life support
- Patient and family teaching techniques
- Strong teamwork and communication skills
- Proficiency or ability to learn Epic electronic medical record
- Understanding of long-term care regulations and resident rights
