Location: Carmichael, California

End Date: 09-22-2026
Contract Type: Contract
Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, 8-hour day shifts
Hours per Week: 40
Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Manage a daily caseload of 5-6 new patients and 3-4 follow-up visits
- Lead meaningful goals-of-care discussions with patients and families
- Educate patients on disease processes, advance directives, and POLSTs
- Facilitate family meetings and provide compassionate end-of-life support
- Review patient charts and assess needs to develop individualized care plans
- Collaborate with physicians and the interdisciplinary team during daily rounds
Qualifications
- Active California RN license
- BLS certification required
- ELNEC certification preferred
- 2-5 years recent palliative or hospice experience
- Acute care experience in ICU or oncology is a plus
- Strong clinical thinking and communication skills

- Cerner electronic medical record system
