Roles & Responsibilities

• Coordinate and manage patient discharges and transitions of care to ensure safe, timely movement to the next level of care
• Perform needs assessments and place orders for DME and community resources as part of a comprehensive plan of care
• Collaborate with interdisciplinary teams, including social services, to remove barriers to discharge and optimize outcomes
• Conduct utilization review and case management activities in line with CMS and facility guidelines
• Document thoroughly and accurately in Epic to maintain continuity of care and regulatory compliance
• Float between hospital areas (including Colby and Pacific campuses) as needed to support patient care across sites

Qualifications

• Active RN licensure (pending licenses accepted) or MSW with relevant acute care experience
• BLS certification required
• Minimum 1 year of acute care experience, with demonstrated discharge planning and care coordination
• Experience with case management, utilization review, and Epic charting
• Comfort working with varied patient age groups (neonates through geriatrics) and patient ratios up to ~1:25 on days
• Willingness to work every other weekend and to float across units/campuses

Education

• Registered Nurse (RN) or Master of Social Work (MSW) — acute care case management experience required

Knowledge & Skills

• Strong knowledge of discharge planning, care coordination, and plan-of-care development
• Familiarity with CMS regulatory expectations and utilization review processes
• Experience ordering and coordinating DME and community services
• Proficiency with Epic electronic charting
• Ability to work in a high-acuity Level II trauma center and collaborate across departments
• Comfort with Med Surg, long length of stay (LLOS), ED and ICU case management is a plus
