Roles & Responsibilities

• Coordinate and manage patient care transitions through thoughtful discharge planning
• Conduct utilization review and needs assessments, including ordering DME when appropriate
• Develop and implement individualized plans of care that align with CMS guidelines
• Collaborate with interdisciplinary teams, social services and community partners to ensure continuity of care
• Document thoroughly in EPIC to maintain accurate, auditable patient records
• Assist with care coordination across units and between the Colby and Pacific campuses

Qualifications

• RN or MSW (RN licensure required for RN role; pending licenses accepted)
• BLS certification required
• Minimum 1 year acute care experience (first-timers accepted with relevant skills)
• Experience in discharge planning, care coordination or case management
• Comfort working across specialties such as Med Surg, ED or ICU is strongly preferred
• Familiarity with EPIC charting system
• Willingness to work every other weekend and float between hospital areas
