Roles & Responsibilities

• Assess and treat speech, language, cognition and swallowing disorders for SNF residents
• Develop individualized treatment plans that support resident goals and safety
• Deliver compassionate, evidence-based therapy while meeting facility productivity expectations
• Collaborate closely with nurses, PT/OT and interdisciplinary team members to coordinate care
• Document sessions accurately and in compliance with CRG policies and SNF regulations
• Educate residents and caregivers on strategies to support communication and swallowing
• Be present on-site to receive guaranteed hours and participate in team activities

Qualifications

• Current state SLP license (required for practice in New Mexico)
• 1-3 years of SLP experience (as requested in the order)
• ASHA CCC-SLP or equivalent certification is a strong plus
• Experience in a skilled nursing facility or with geriatric populations preferred
• Comfortable following facility policy & productivity expectations (90%)
• Ability to work on-site and follow dress code (black scrubs)

Knowledge & Skills

• Skilled in clinical assessment and treatment planning for adult/geriatric populations
• Competence in dysphagia screening and management within SNF protocols
• Strong documentation skills and familiarity with SNF/rehab workflows
• Effective interdisciplinary communication and teamwork
• Time management to balance caseload and productivity requirements
