Roles & Responsibilities

Provide direct nursing care to clinic patients within the scope of LPN licensure, ensuring safe, high-quality patient interactions
Take and record vital signs accurately and perform focused patient assessments
Triage patient calls and determine appropriate next steps, coordinating with providers as needed
Process medication refill requests and complete prior authorizations in a timely manner
Serve as primary liaison between patients, clinical staff, and providers to facilitate smooth care delivery
Give direction to CNAs and schedulers and work collaboratively with RNs, providers, coordinators, and clinical managers

Qualifications

Current State LPN license (must be active and in good standing)
BLS (Basic Life Support) certification required
Graduate of an accredited nursing program (LPN)
Comfortable performing triage, taking vital signs, managing medication refills, and completing prior authorizations
Strong interpersonal and communication skills; able to collaborate with interdisciplinary teams and provide direction to CNAs

Education

Graduate of an accredited LPN program

Knowledge & Skills

Triage protocols and urgent care workflow
Vital signs assessment and basic patient assessment
Medication refill processing and prior authorization procedures
Clinical documentation and patient communication
Ability to coordinate with schedulers and supervise CNAs
