Roles & Responsibilities

• Consult with staff and patients on skin prevention, wound care, ostomy management, and continence needs
• Perform patient assessments and develop individualized care plans focused on healing and quality of life
• Provide expert ostomy and wound care consultations to interdisciplinary teams
• Educate patients, families, and staff on wound prevention, ostomy care, and continence strategies
• Document clinical findings and care plans accurately and professionally
• Serve as a patient/staff liaison to promote clear communication and coordinated care

Qualifications

• Licensed as a Registered Nurse in the State of Pennsylvania (required)
• Board certification in Wound, Ostomy and Continence Nursing (WOCN) (required)
• Current BLS certification that meets American Heart Association Healthcare Provider standards
• Minimum of 3 years full-time professional nursing experience with medical-surgical patients
• Strong communication skills and the ability to work collaboratively with interdisciplinary teams

Education

• Completion of an accredited Registered Nurse degree program (ADN or BSN) required
• High school diploma or equivalent noted as baseline

• Wound care assessment and treatment

• Patient assessment and individualized care planning
• Clinical documentation and patient education
• Excellent verbal, non-verbal and written communication
• Teamwork and effective liaison skills with staff and families
