Physical Therapist - Home Health
Location: Sunnyvale, California
Start Date: 06-15-2026
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:30 PM (flexible based on patient availability), One weekend day per month
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Restore function and minimize disability for patients recovering from illness or injury
- Evaluate and treat patients using medically prescribed physical therapy programs
- Supervise Physical Therapy Assistants and guide their clinical development
- Conduct initial admission and evaluation visits with compassion and expertise
- Document observations, clinical findings, and skilled interventions to support quality care
- Collaborate with the clinical team to help patients reach their maximum potential
Qualifications
- Active Physical Therapist license in California
- Current BLS/BCLS certification
- Minimum one year of Home Health experience
- Strong assessment and documentation skills

- Home Health physical therapy expertise
- OASIS documentation proficiency
- Patient-centered care approach
- Ability to work independently in diverse home settings
