Physical Therapist - Home Health
Location: Sunnyvale, California
Start Date: 05-27-2026
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:30 PM (Schedule can be flexible based on patient's availability)
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Restore function and minimize disability following disease or injury
- Help clients reach their maximum performance within their capabilities
- Evaluate and treat patients using medically prescribed physical therapy programs
- Supervise Physical Therapy Assistants as needed
- Conduct initial admission and evaluation visits
- Document observations, clinical findings, skilled interventions, goals, and discharge plans
- Notify Clinical Team Manager of OASIS scores within established timelines
Qualifications
- Active Physical Therapist license in California
- BLS/BCLS certification required
- One year of Home Health experience required
- Strong documentation and clinical assessment skills

- Physical Therapy evaluation and treatment techniques
- Home Health care delivery
- OASIS documentation and scoring
- Patient-centered care planning
- Supervision of Physical Therapy Assistants
