Physical Therapist - Home Health
Location: Santa Maria, California
Start Date: 05-27-2026
End Date: 08-26-2026
Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Deliver personalized physical therapy services in patients' homes across your assigned region
- Evaluate and treat a diverse caseload including ortho, stroke, joint replacements, and spinal injuries
- Complete OASIS documentation accurately and on time to meet regulatory requirements
- Collaborate with PTAs, OTAs, and rehab aides to provide comprehensive care
- Build meaningful relationships with patients and families while helping them regain independence
Qualifications
- Active Physical Therapist license in California
- BLS certification required
- At least one year of clinical experience as a Physical Therapist
- Prior experience with OASIS documentation
- Home health experience is preferred but not required
Knowledge & Skills
- Strong assessment and treatment planning skills for home-based patients
- Comfortable using Cerner EMR for documentation
- Excellent communication and patient education abilities
- Ability to work independently while thriving as part of an interdisciplinary team
- Flexible and adaptable approach to scheduling and patient needs
