Physical Therapist

Location: Glendale, CA

Contract Type: Per-Diem

Shift Type: PRN / Per Diem, flexible scheduling based on facility needs (shifts vary; contact the team for available shift days and specific start/end times)

Candidate Pay Rate: $85 - $59.5 per hour

Roles & Responsibilities

Evaluate and assess patients to develop individualized, outcome-focused treatment plans
Deliver therapeutic interventions to improve mobility, strength, and functional independence
Collaborate with interdisciplinary team members including OT, SLP, and nursing to coordinate care
Monitor patient progress and adapt treatment plans to maximize recovery
Maintain accurate, timely documentation in compliance with facility and regulatory standards
Educate patients and caregivers on treatment goals and home exercise programs to support long-term gains

Qualifications

Active California Physical Therapist (PT) license (required)
Previous SNF or geriatric experience preferred
Strong clinical reasoning and documentation skills
Able to work independently and as part of a collaborative team
Flexibility to pick up shifts as needed and adapt to facility scheduling

Knowledge & Skills

Skilled in physical therapy assessment and treatment planning
Experience improving mobility, strength, and functional independence
Proficient with clinical documentation and regulatory compliance
Able to communicate effectively with patients, families, and interdisciplinary teams
Comfortable teaching home exercise programs and patient education
