Roles & Responsibilities

Provide compassionate bedside care to residents, focusing on dignity and comfort
Administer medications and treatments accurately and document appropriately
Monitor and report changes in resident condition to the care team promptly
Collaborate with nurses, CNAs, and therapists to deliver coordinated care
Assist with admissions, discharges, and care planning activities
Maintain clean, safe environments and follow infection control protocols

Qualifications

Current LPN license (Pennsylvania license or ability to obtain one)
Completion of an accredited practical nursing program
Basic Life Support (BLS/CPR) certification preferred or willing to obtain
Comfortable with electronic charting and documentation
Reliable, professional, and able to pass background and health screenings
Strong communication and teamwork skills; patient-centered attitude

Education

Graduate of an accredited LPN / Practical Nursing program
High school diploma or equivalent

Knowledge & Skills

Medication administration and safe documentation practices
Basic assessment skills and recognizing changes in patient status
Familiarity with long-term care or skilled nursing routines
Good time management and prioritization during busy shifts
Clear communication with residents, families, and interdisciplinary teams
