End Date: 08-17-2026

Contract Type: Travel Contract

Shift Type: 4x10-Hour Day shifts, 8:00 AM - 6:30 PM, every other weekend

Hours per Week: 40

Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Coordinate seamless patient care transitions through expert discharge planning
Assess patient needs and order essential DME to support recovery at home
Develop comprehensive plans of care that make a real difference in high-volume settings
Champion care coordination across interdisciplinary teams including social services
Navigate regulatory landscapes like CMS, HIPAA, and Joint Commission standards with confidence
Solve complex problems creatively while managing patient care in a dynamic acute hospital environment

Qualifications

Active CA RN License
AHA BLS certification
Minimum 2 years recent acute hospital experience
Proven discharge planning experience
COVID-19 vaccine required plus booster, medical/religious exemptions only

Education

Master's Degree preferred

Knowledge & Skills

Resourcefulness and creative problem-solving
Superior written and oral communication
Diplomacy in high-volume work environments
Strong organizational and analytic skills
Expertise in systems planning and patient care management
Familiarity with care coordination, worker's comp case management, CMS, HIPAA, and Joint Commission standards
