Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Deliver compassionate one-on-one physical therapy in patients' homes
Conduct up to 6 patient visits per day with 30 productivity points
Develop and implement home exercise programs for lasting recovery
Educate patients and families on body mechanics and self-care
Assess and treat orthopedic and neurologic conditions with expertise
Minimize travel time through smart client grouping by the facility

Qualifications

Active Physical Therapy license in Oregon (pending accepted)
BCLS/BLS certification required
Oasis charting experience (must have or be proficient)
Own reliable transportation for home visits
Primary COVID series plus one booster

Education

Physical Therapy degree from accredited program

Knowledge & Skills

Home health therapy delivery
Oasis and EPIC charting
Orthopedics including post-op surgeries, spinal conditions, hip/knee replacements
Neurologic conditions like stroke rehab, Parkinson's, spinal cord injury
Patient and family education
Body mechanics and assessment expertise
