Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (5x8-hour day shifts)

Hours per Week: 40

Candidate Pay Rate: $67 per hour

Roles & Responsibilities

Deliver skilled physical therapy in patients' homes, making a real difference in their daily lives
Perform comprehensive assessments including OASIS documentation and vital signs monitoring
Develop and implement personalized care plans with home exercise programs
Provide hands-on treatments like gait training, wound care, and modality therapies
Educate patients and families on safety, injury prevention, and self-management
Coordinate schedules, order supplies, and manage up to 6 patient visits per day

Qualifications

Active Oregon PT license
BCLS certification
1 year of skilled home health experience (first-timers welcome)
OASIS experience required
Own reliable transportation
COVID primary series plus one booster

Education

Physical Therapy degree from accredited program
Eligible for Oregon PT licensure

Knowledge & Skills

Expertise in orthopedics, geriatrics, neurology, and wound care
Proficient with EPIC Remote Client, Rover app, and OASIS
Skilled in lifting/transfers, gait training, prosthetic training, and modalities like e-stim and ultrasound
Strong assessment skills for pain, strength, skin integrity, and environmental barriers
Patient education on medications, home exercises, and safety
