Candidate Pay Rate: $77 per hour

Roles & Responsibilities

Provide comprehensive wound care and IV therapy to patients in their homes
Educate patients and families on managing chronic conditions like CHF and COPD
Coordinate care and monitor patients recently discharged from hospital to prevent rehospitalization
Perform start of care assessments, recertifications, and routine visits
Collaborate with home health aides and on-call support team

Qualifications

Valid BLS certification required
Valid driver's license and auto insurance required
Home health nursing experience preferred
HCHB (HomeCare HomeBase) experience preferred

Knowledge & Skills

Wound care and wound vac management
IV therapy and PICC line care
Patient and family education
CHF, COPD, wound care, and other common home health diagnoses
Time management and ability to handle schedule changes
