Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Perform start of care, recertifications, and resumption of care visits
Conduct regular patient visits (5-6 per day) and admissions as needed
Develop and implement individualized treatment plans
Collaborate with clinical coordinators and sister agencies for support
Complete OASIS documentation accurately and timely
Provide safety evaluations and teaching visits in hospice situations as needed

Qualifications

Active ND Physical Therapy License required
Current BLS certification
Valid Driver's License and auto insurance
Home Health experience strongly preferred
OASIS training preferred
First-time travelers welcome to apply

Knowledge & Skills

Proficiency in OASIS documentation
Strong clinical assessment and decision-making skills
Excellent communication and patient education abilities
Time management and adaptability in a field setting
Ability to work independently with remote support
