Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Provide exceptional physical therapy services to patients in their homes across four counties (Noble, Lagrange, Steuben, DeKalb)
Conduct Oasis Assessments and Start of Care evaluations
Develop personalized care plans to help patients regain independence
Accurately document patient progress using a provided tablet
Collaborate with a dedicated healthcare team to ensure holistic care

Qualifications

Active Physical Therapist license in Indiana
Experience with Oasis Assessments and Start of Care
Home health experience preferred but not required
Valid driver's license and reliable transportation for rural travel
Ability to work independently and manage your schedule

Knowledge & Skills

Strong clinical assessment skills
Proficiency in Oasis documentation
Knowledge of home health regulations
Excellent time management and organizational skills
Compassionate patient communication
