Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Yolo County
Assess and evaluate each patient's therapeutic needs and develop personalized care plans
Guide patients and families through rehabilitation exercises and the proper use of therapeutic equipment
Monitor progress and communicate changes in condition to physicians promptly
Collaborate with home health aides and caregivers to ensure consistent, high-quality care
Document clinical notes and progress summaries that support patient goals and regulatory compliance

Qualifications

Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and rehabilitation planning
Knowledge of Title 22, Medicare, and Medi-Cal regulations
Ability to work independently while staying connected with the care team
