Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes
Help patients recover from surgeries, falls, and other conditions with compassion
Collaborate with home health and hospice teams to support whole-person healing
Document care using HomeCare/HomeBase EMR for seamless coordination
Travel within a 60-mile radius to bring expert therapy directly to patients

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of previous home health experience
Valid driver's license and auto insurance
Strong PT-related clinical skills

Home health physical therapy expertise
HomeCare/HomeBase EMR proficiency
Compassionate, patient-centered approach
Ability to work independently in home settings
Excellent communication and documentation skills
