Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Tehama County
Conduct Start of Care visits for orthopedic patients when nursing is not ordered
Develop personalized treatment plans for geriatric patients with multiple comorbidities including CHF and COPD
Complete approximately 5 meaningful patient visits each day using the HCHB EMR system
Collaborate with the intake RN and office manager to coordinate exceptional patient care
Travel throughout the service area while enjoying a tablet provided for seamless documentation

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years of experience who have Home Health background
Strong assessment and documentation skills

Knowledge & Skills

Physical Therapy expertise in home health settings
Experience with orthopedic and joint replacement patients
Comfort working with geriatric populations and complex medical conditions
Proficiency with EMR systems, preferably HCHB HomeCare HomeBase
Excellent time management and independent decision-making abilities
