Contract Type: Travel Contract

Shift Type: Day shift, Monday-Friday, no weekends or holidays required

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive home health assessments for orthopedic and geriatric patients
Develop and implement individualized physical therapy treatment plans
Provide skilled therapy services in patients' homes across Tehama County
Collaborate with nursing and therapy team for coordinated patient care
Document patient progress in HCHB (HomeCare HomeBase) EMR system
Educate patients and families on home exercise programs and safety

Qualifications

Active Physical Therapist license in California (or compact eligible)
BLS certification required
At least 2 years of experience preferred; will consider travelers with less if they have home health experience
Experience with HCHB (HomeCare HomeBase) EMR is a plus

Knowledge & Skills

Home health and therapy best practices
Orthopedic joint replacements and geriatric comorbidities (CHF, COPD)
Patient assessment and treatment planning
Time management and self-scheduling
Compassionate communication with patients and families
