Roles & Responsibilities
- Assess and treat patients in their homes to help them regain independence in daily activities
- Collaborate with physicians and healthcare team members to develop personalized care plans
- Provide supportive supervision to OTAs and OT/OTA students
- Maintain accurate and timely documentation in the electronic medical record
- Promote a clean and safe environment using SAFE error prevention habits
Qualifications
- At least 1 year of Home Health OT experience
- Valid driver's license and car insurance
- Active Illinois Occupational Therapy license
- COVID-19 vaccination (declinations not approved)
- OASIS and start of care experience strongly preferred
- Current BLS certification
Knowledge & Skills
- Occupational therapy assessment and treatment planning
- Evidence-based practice and patient-centered care
- Strong interpersonal and communication skills
- Proficient in documentation and electronic medical records
- Ability to transfer and position patients safely
