Roles & Responsibilities

• Provide compassionate nursing care to residents in a skilled nursing facility
• Administer medications and treatments as prescribed by physicians
• Monitor patient vital signs and report changes to the healthcare team
• Collaborate with nurses, doctors, and aides to ensure quality patient care
• Maintain accurate and timely patient records and documentation

Qualifications

• Active LPN license in Vermont required
• BLS certification required
• Minimum 1 year of experience in a skilled nursing or long-term care facility preferred
• Must have own reliable transportation
• Ability to work every weekend and night shifts

Education

• Graduation from an accredited LPN program

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Comfortable working independently and as part of a collaborative team
• Proficiency in electronic health records and documentation
