Roles & Responsibilities

• Provide compassionate direct patient care and support daily living needs
• Administer medications and treatments safely and accurately according to orders
• Monitor and document patient vital signs and condition changes in the chart
• Collaborate with nurses, therapists, and interdisciplinary team members to deliver coordinated care
• Educate patients and families about care plans, medications, and follow-up
• Maintain accurate electronic health records and ensure timely documentation
• Follow infection control and safety protocols to protect patients and staff
• Participate in shift handoffs and communicate critical information clearly

Qualifications

• Active LPN license in Pennsylvania or eligible for Pennsylvania LPN licensure
• Current BLS/CPR certification (required)
• Clinical experience in long-term care, rehab, or acute settings preferred
• Strong communication skills and ability to work well within a team
• Ability to pass background check and drug screening per facility policy

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program (diploma or certificate)
• High school diploma or GED required

Knowledge & Skills

• Solid knowledge of medication administration and documentation
• Basic wound care and dressing change familiarity
• Patient assessment, vital sign measurement, and clinical observation
• Comfortable with electronic health record charting
• Good time management and the ability to prioritize tasks during a shift
• Professional bedside manner and patient-centered communication
