Roles & Responsibilities

• Provide safe, compassionate basic nursing care to residents in accordance with facility policies
• Administer medications and treatments as ordered, documenting accurately
• Assess resident conditions and promptly communicate changes to the care team
• Support resident hygiene, mobility, and daily living activities with dignity
• Collaborate with nurses, therapists, and support staff to create person-centered care plans
• Maintain accurate and timely documentation in electronic or paper records
• Adhere to infection control standards and promote a clean, safe environment

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania or ability to obtain prior to start
• Current CPR/BLS certification (or willingness to obtain promptly)
• Prior experience in long-term care, skilled nursing, or similar clinical setting preferred
• Strong communication skills and a team-oriented attitude
• Ability to pass required background checks and health screenings

Education

• Graduate of an accredited Practical Nursing program or equivalent vocational training

Knowledge & Skills

• Knowledge of basic nursing procedures, medication administration, and wound care
• Skill in resident assessment and recognizing changes in condition
• Familiarity with documentation best practices and charting accuracy
• Ability to work efficiently under routine clinical workflows and follow protocols
• Good interpersonal skills for working with residents and interdisciplinary teams
