Roles & Responsibilities

Provide compassionate direct patient care, including basic assessments and monitoring vital signs
Administer medications and treatments safely and accurately according to plan
Document patient information clearly and timely in the medical record
Assist patients with activities of daily living and promote their dignity and comfort
Collaborate with nurses, providers, and the care team to coordinate patient care
Educate patients and families about care plans and follow-up instructions
Maintain infection control practices and a safe, clean environment
Respond calmly and effectively in urgent situations to support patient stability

Qualifications

Valid LPN license in Pennsylvania or eligibility to obtain licensure
Current BLS (Basic Life Support) certification or willingness to obtain
Comfortable with medication administration, vital signs, and basic patient assessments
Strong communication and teamwork skills; dependable and patient-centered
Previous experience in a clinical or long-term care setting is a plus

Education

Completion of an accredited LPN/LVN program
High school diploma or equivalent

Knowledge & Skills

Patient assessment and monitoring
Medication administration and documentation
Charting and record-keeping best practices
Infection prevention and safety protocols
Effective patient and family communication
Basic wound care and ADL assistance
