Roles & Responsibilities

• Provide compassionate, person-centered care to residents and patients
• Administer medications and treatments accurately, following policy
• Perform regular patient assessments and promptly report changes to the RN or provider
• Assist residents with activities of daily living (ADLs) while preserving dignity
• Document care thoroughly and maintain accurate charts in the clinical record
• Collaborate with nursing staff, therapists, and interdisciplinary team members to support care plans
• Support infection control practices and maintain a safe care environment
• Contribute to a positive resident experience through professional and friendly interactions

Qualifications

• Current, active LPN license in Pennsylvania (required)
• Completion of an accredited practical nursing program or equivalent
• Basic Life Support (BLS) / CPR certification (preferred or required per facility policy)
• Recent clinical experience as an LPN, ideally in long-term care or skilled nursing (preferred)
• Strong communication skills and the ability to work well within a team

Education

• Vocational nursing diploma or certificate from an accredited LPN program (required)

Knowledge & Skills

• Medication administration and documentation best practices
• Vital signs monitoring and basic patient assessment
• Charting and electronic health record (EHR) familiarity
• Infection control principles and safe patient handling
• Time management, prioritization, and teamwork
• Empathy, patience, and excellent resident-facing communication
