Roles & Responsibilities

Provide compassionate direct patient care with attention to safety and dignity
Administer medications and treatments accurately following facility protocols
Monitor patient status, document care thoroughly, and report changes promptly
Collaborate with the care team to coordinate patient needs and discharge planning
Support a positive patient experience through clear communication and empathy
Maintain compliance with regulatory and facility standards for nursing practice

Qualifications

Current LPN license (PA license strongly preferred)
Active CPR/BLS certification (or willingness to obtain)
Previous LPN experience in a clinical or long-term care setting preferred
Strong communication skills and the ability to work well within a multidisciplinary team
Up-to-date immunizations and ability to pass required background checks

Education

LPN diploma, certificate, or equivalent practical nursing education

Knowledge & Skills

Patient assessment and basic nursing care skills
Medication administration and documentation
Vital signs monitoring and basic clinical observation
Electronic charting and written communication skills
Team collaboration, time management, and patient advocacy
