Roles & Responsibilities

Provide compassionate, hands-on patient care to residents, making every day safer and more comfortable
Administer medications and treatments accurately and on schedule
Monitor and document vital signs, patient responses, and changes in condition
Collaborate with nurses, therapists, and support staff to create and follow meaningful care plans
Maintain accurate and timely patient records in the charting system
Support residents with activities of daily living while promoting dignity and independence
Follow infection control and safety procedures to protect patients and staff

Qualifications

Current LPN license (Licensed Practical Nurse) — Pennsylvania licensure required
CPR/BLS certification preferred or obtained upon hire
Comfortable working 12-hour clinical shifts and delivering consistent, high-quality care
Strong communication skills and a team-player attitude
Previous experience in long-term care or similar settings is a plus

Education

Completion of an accredited LPN/LVN program (vocational nursing certificate/diploma)
High school diploma or GED

Knowledge & Skills

Medication administration and basic clinical procedures
Patient assessment and monitoring
Charting and documentation best practices
Infection control and safe patient handling
Clear communication with patients, families, and interdisciplinary teams
Time management and prioritization during busy clinical shifts
