Roles & Responsibilities

Provide compassionate bedside care to residents and patients with professionalism and respect
Administer medications and treatments accurately and safely according to care plans
Monitor patient status and report changes promptly to the interdisciplinary team
Document assessments, interventions and patient responses clearly in the medical record
Collaborate with nurses, physicians and ancillary staff to deliver coordinated care
Educate patients and families about care plans, medications and follow-up needs

Qualifications

Valid Pennsylvania LPN license required
Graduate of an accredited practical nursing program
Current BLS/CPR certification (or willingness to obtain)
Demonstrated clinical judgment and strong communication skills
Ability to pass background and health screening per facility policy
Previous experience in long-term care or similar clinical setting preferred

Education

Completion of an accredited LPN (Practical Nursing) program

Knowledge & Skills

Medication administration and basic clinical procedures
Patient assessment and vital sign monitoring
Electronic health record documentation
Infection control and safety protocols
Strong prioritization, time management and teamwork
Compassionate patient-centered care and effective communication
