Roles & Responsibilities

• Provide compassionate, high-quality bedside care to residents using your LPN skills
• Administer medications and treatments accurately and document in the medical record
• Monitor and report changes in patient condition to the interdisciplinary team
• Collaborate with nurses, CNAs, therapists, and leadership to deliver individualized care plans
• Maintain a safe, clean environment and follow infection control protocols
• Support residents' emotional needs and communicate respectfully with families

Qualifications

• Active LPN license in Pennsylvania or ability to obtain licensure prior to start
• Current Basic Life Support (BLS/CPR) certification (or willingness to obtain)
• Ability to pass background checks and required health screenings/immunizations
• Strong clinical judgment, time management, and teamwork mindset
• Previous long-term care or skilled nursing experience preferred but not required

Education

• Graduation from an accredited LPN/Vocational Nursing program
• High school diploma or equivalent

Knowledge & Skills

• Clinical skills: medication administration, wound care basics, vital signs monitoring
• Documentation: accurate charting and familiarity with electronic medical records
• Communication: clear, compassionate interactions with residents, families, and staff
• Regulatory awareness: understanding of basic nursing standards and infection control
• Teamwork: collaborate effectively across disciplines to achieve excellent resident outcomes
