Contract Type: Per-Diem

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care, focusing on dignity and comfort
Administer medications accurately and document administration according to facility policy
Monitor patients for changes in condition and communicate findings promptly to the care team
Assist with activities of daily living and promote patient independence
Support admission, transfer and discharge processes to ensure smooth transitions
Collaborate with RNs, physicians and interdisciplinary staff to deliver coordinated care
Maintain accurate and timely clinical documentation in patient records

Qualifications

Current LPN license in good standing (required)
Basic Life Support (BLS/CPR) certification or willingness to obtain
Ability to pass background check and required health screenings
Comfortable with medication administration, wound care basics, and routine clinical tasks
Strong communication and teamwork skills; patient-centered attitude

Knowledge & Skills

Clinical patient assessment and basic nursing interventions
Medication administration and documentation practices
Familiarity with long-term care or skilled nursing workflows
Good interpersonal communication and ability to educate patients and families
Basic electronic charting / EHR navigation skills
